Celebration of Literacy Nomination Form
We welcome you to nominate outstanding students, volunteer tutors, programs, businesses, and advocates to be honored at Wisconsin Literacy’s 12th Celebration of Literacy at the Monona Terrace Community and Convention Center in Madison on Wednesday, April 25, from 11:00 a.m. to 1:15 p.m.
Please nominate only those who can attend the award ceremony, can send a representative, or provide a video acceptance speech. You may submit multiple nominations by completing a separate nomination form for each nominee (although only one award will be made per member agency). Each winner receives a plaque and two complimentary tickets to the Celebration. All nominees will be recognized during a nominee slideshow.
To make a nomination, review the award categories and fill in the form or send a 2-3 minute video using the form questions. Submit your nominations no later than Friday, January 26th. In an effort to streamline the nomination process, we’ve created broad award categories.
Note: There may be multiple awards given within each of the three categories below
Outstanding Literacy Learner
Nominate a learner (Adult Basic Education, English Language, Family Literacy, Correctional Setting, HSED/GED student, etc.) in recognition of their significant achievements as a result of increased literacy.
Nominate an outstanding or innovative literacy program (family, health literacy, or workplace, etc.) The program should demonstrate improved literacy outcomes that greatly improve the lives of learners.
Literacy Service Recognition
Nominate a volunteer tutor and/or literacy advocate (elected official, agency professional, or member of the public) who demonstrates a commitment to literacy through tutoring and mentoring his/her learner(s) and/or who has advocated on behalf of adult learners, families, and the programs that serve them.
